Committee of Department Success Report Form

The Department Success report form was created to allow students, staff and faculty to submit problems and grievances to the Committee of Department Success of the Department of Geoscience at UNLV. The Committee of Department Success will review and discuss your submission in their next committee meeting, unless specified otherwise.

This is an anonymous form. If you would like, you can share your contact information, as well as let the Committee know if you would like them to reach out to you to discuss further.

Report Form

Note, if you have any problems or grievances regarding the department and its members, you have different options:

  • Contact the chair of the department
  • Contact the chair of the Committee of Department Success, or any other members of the Committee.
  • Contact any faculty member
  • Contact any resources on campus, which can be found here
  • Fill out the Report Form provided.
